How-To: Get Started

Survey Says...  Lisa Kihn

Here is a survey that you can give to parents the first week of school. This questionnaire will provide you with invaluable information about the students in your class. I use this information as a way to get to know parents and students better. Many parents will provide you with a unique insight into their child and you can use this information to help you meet individual learning needs. I often refer to this survey during parent teacher conferences and use it as a way to begin a meaningful conversation with parents about their children. I also find it helpful to know parent's professions in case I need an expert in that field to volunteer in the class.

Parent Survey

Please take a few minutes to provide us with the following information about your child.

1. Child's name __________________________________________

2. Parent(s) occupation _______________________________________________

3. Who does your child live with? _______________________________________

4. What are some things that your child likes to do outside of school?

5. What school activities does your child seem to enjoy most and what activities does your child enjoy least?

6. What are some goals that you would like to see your child accomplish this year?

7. What else would you like for me to know about your child that will help to make school a better experience for him/her this year? Please feel free to write as much as you like (use the back or attach extra sheets).
